Instructions for Completing Paperwork

 

Filling out the paperwork properly will assist us in meeting licensing requirements, so please read the information below to ensure that you are correctly filling the paperwork out.  If you have questions, please feel free to let us know and we can assist you.

 

Enrollment Form

Please fill in each box of the form.  If an area does not apply to you, write not applicable.  If at any time this information has changed please see the director to make appropriate changes. Download Enrollment Form Here

Health History

Immunization Form

You can fill in the information, or you can give a copy of the most recent immunization record.  Please share an updated record each time your child receives a new shot, unless it is the flu shot. Download Immunization Form Here

Health Form

This form needs to be filled out by a doctor.  We must have this on file for each child unless they are a school-aged child (5 years and up).  For children under the age of 2, this form must be completed again every six months.  For children 2 years and older, the form must be completed every 2 years.  Please have the doctor indicate if your child needs special formula/food.  For example: child is allergic to milk please give soy milk. Download Health Report Form Here

Intake Form

This form only needs to be filled out if your child is aged 2 and under.  This form must be updated in the child’s classroom every 3 months. Download Intake Form Here

Alternate Arrival and Release Form

This form must be completed if your child is arriving to the center via another way versus by parents or other approved drop off persons.  For example, if your child arrives by school bus, this form must be completed. Download Alternate Arrival and Release Form Here

Parent Manual

After you have read the manual, there is a page in the back that you can sign off on letting us know that you understand our policies.  Please turn this page only back into the office. Download Parent Manual Here
